Notice: There are two updated consent forms under the "Consents and Questionnaires" tab that all of our patients are required to sign.  One is titled "Fee-For-Service Agreement (02/2025)" which is an updated consent form for our new pricing schedule, and the other consent form is titled "Credit Card Authorization Form" which is an updated authorization form for credit card charges.  Please sign these forms at your earliest convenience.  Thank you! 


Pharmacies sometimes have delays in processing orders.  Please continue to allow 2 weeks for delivery.  But, be sure to let us know if you have not received your order within 2 weeks of our order.  


Due to increased patient volume, rescheduling of regularly scheduled appointments is becoming a problem for those who do not complete their labs in a timely manner.  Please keep in mind that rescheduling a follow-up appointment can now take up to up to 30 days.  An offered reschedule appointment will be held for 24 hours.  After 24 hours it may be offered to another patient.  Please remember that Dr. Phillips does not refill prescriptions between regularly scheduled appointments.
